CHAN has a position for a Clinical Quality Improvement Director/Workflow Analyst to oversee the administration of the clinical quality improvement program. The position includes but is not limited to: Best Practices, QI, Meaningful Use and Patient Centered Medical Home QI initiatives, Health Center Controlled Network (HCCN) funded QI initiatives, continuous improvement initiatives. Develops appropriate strategies and best practices, to include workflow analysis, and provides leadership for improving quality programs in support of clinical operations and clinical programs.
*reports to the Executive Director and advisory relationship with CHAN Health Services Committee and HCCN Steering Committee and is responsible for leading and evaluating a comprehensive Network Collaborative quality initiative to exceed the HealthyPeople 2020 goal.
*Support Network Collaborative participants in MU and PCMH quality initiatives, to include workflow analysis, as it relates to existing EHR systems.
*Perform complex data analysis in support of Network Collaborative quality initiative(s) and develop programs, methodologies, and files for presenting data.
*Lead new quality initiatives
*Reports to the CHAN Board of Directors and HCCN SC regarding performance improvement plan bi-yearly.
*Coordinates the integration of a "dashboard" for clinical outcomes, critical interventions, cost, utilization, satisfaction, etc.
*Ability to work collaboratively with staff to improve performance and lead Practice redesign for clinical quality.
*Minimum of four years work experience in health setting or degree in health or human services with appropriate experience. Extensive experience with information management practice redesign.
Salary range $68,000 to $82,000 depending upon experience.
