The LCSW provides professional counseling, therapy, and other clinical services of a preventative and developmental nature through direct consultation with patients. May be responsible for supervising the work of Social Workers, Interns, or other caseworkers. Responsible for integrating clinical counseling services with overall patient care through assessing the patient¿s psychosocial needs and for planning, implementing, and coordinating care through the continuum of care (clinic, hospital, and transition to the community). Works as part of a multidisciplinary team.
DEMONSTRATES ABILITY TO INTEGRATE CLINICAL COUNSELING SERVICES WITH OVERALL PATIENT CARE THROUGH THE ASSESSING OF THE PATIENT¿S PSYCHOSOCIAL NEEDS AND PLANNING FOR, IMPLEMENTING, AND COORDINATING CARE THROUGH THE CONTINUUM OF CARE (CLINIC, HOSPITAL AND TRANSITION TO THE COMMUNITY).
*Evaluates patients through interviews and comprehensive biopsychosocial assessments.
*Develops and implements an individualized treatment approach to meet the needs of each patient and their families.
*Works in collaboration with all members of the treatment team, including physicians, to further their understanding of significant social and emotional factors contributing to the patient¿s well being.
DEMONSTRATES ABILITY TO PROVIDE PROFESSIONAL COUNSELING AND CLINCIAL SERVICES OF MEDIATION, COUNSELING, AND DIRECT PRACTICE THERAPY. THERAPY ACTIVITIES INCLUDE INDIVIDUAL, GROUP AND COUPLES COUNSELING. SCOPE INCLUDES, BUT IS NOT LIMITED TO DIAGNOSTIC ASSESSMENT, COUNSELING, PSYCHOTHERAPY, BRIEF THERAPY, BEHAVIOR MODIFICATIONS, AND MENTAL HEALTH COUNTULTATION.

*Provides counseling services involving social work assessments, counseling, clinical therapy, and/or treatment to patients, significant others, or group setting.
*Develops and provides stress management/wellness treatment plans for inpatient and outpatient populations.
DEMONSTRATES ABILITY TO PROVIDE INFORMATION AND REFERRAL SERVICE AND PROBLEM SOLVING.
*Identifies and provides outreach in the form of information and referrals to community resources to patients and families such as shelter, transportation, and financial assistance.
*Facilitates Long-Term Acute and Chronic Care placement and other facility transfer arrangements.
*Facilitates availability of Durable Medical Equipment and Expendable supplies.
DEMONSTRATES ABILITY IN INSTRUCTING AND SUPERVISING SOCIAL WORKERS, INTERNS AND OTHER CASE MANAGERS.
*Provides supervision of LBSW staff, LMSW staff, other case manages, and field supervision of students from Baccalaureate and Graduate Schools of Social Work.
*Collaborates with teaching programs with social work degree programs, schools of medicine, and nursing school programs.
*Demonstrates initiative and leadership in developing services which respond to patient care demands.
*Provides social work practice based upon continuous quality improvement.
*Assess patient and family advocacy needs, and coordinate advocacy activities within the hospital and the community with the emphasis on client self-determination.
MAINTAINS ACCURATE, TIMELY AND APPROPRIATE CLINICAL RECORDS AND ADHERES TO THE TEXAS STATE SOCIAL WORK CODE OF ETHICS.
*Gathers information in a thorough manner from all resources in order to fully document current patient status and prognosis.
*Accurately and clearly documents all patient records and electronic data submissions in a timely and professional manner.
PERFORMS OTHER POSITION APPROPRIATE DUTIES AS REQUIRED IN A COMPETENT, PROFESSIONAL AND COURTEOUS MANNER.
SERVES AS A MEMBER OF THE TRANSPLANT MULTIDISCIPLINARY TEAM.
*Participates in department meetings and provides input to improve the process of the team.
*Participates in transplant team building activities.
*Collaborates with Financial Coordinator and Transplant Coordinator regarding patient issues.
*Coordinates discharge planning for patients after transplant or hospital stays.
SERVES IN ACTIVITIES RELATED TO ORGAN DONATION.
*Participates in organ donation awareness initiatives sponsored by the hospital or other entitities.
*Participates with community advocacy groups that deal with kidney health and donation.
PARTICIPATES IN ACTIVITIES TO PROMOTE PATIENT EDUCATION AND SUPPORT SERVICES.
*Actively participates with the transplant support group and newsletter.
MAINTAINS ACCURATE TIME STUDIES.
*Participates in daily time studies for the Kidney Acquisition Cost Report.
REQUIRED SKILLS, KNOWLEDGE AND ABILITY:
*Demonstrates oral and written mastery of the English language.
*Two to three years experience desired.
*Training or experience in clinical social work skills of counseling and therapy as well as bereavement and/or case management desired.
*Demonstrates recognition of patient¿s right to privacy and confidentiality as well as understanding the record needs of the organization.
*Takes personal responsibility to understand, complete, and maintain the professional and technical licenses required for the position and furnishes Scott & White with the necessary documentation to substantiate compliance.
*Transplant Center experience desired.
