Responsible for identification and education of all referral sources in the PACE of Southwest Michigan service area. The person in this position reports to the Executive Director and is responsible for assisting in educating the healthcare community and general public regarding the full spectrum of PACE of Southwest Michigan services. This includes meeting with physicians, hospital discharge planners, leaders in the various post-acute facilities in the service area, and other potential referral sources in the healthcare community. Additionally, the person in this position will assist with community education through participation in community events and speaking engagements. The Community Relations Representative is responsible for assisting the intake team in meeting budgeted goals for enrollments. The Community Relations Representative is responsible for developing and sustaining referral relationships and will determine the most effective and efficient way for interfacing with each referral source. Positive communication and optimal coordination with Intake, Center Manager, Medical Director and Executive Director will be essential to assure continuity of efforts to meet the needs of referral sources.
Acts as a representative of PACE of Southwest Michigan to the community through direct involvement, presentations, vendor fairs and other activities. Identifies key personnel at existing and potential referral sources and makes ongoing contact to build effective relationships. Maintains schedule of regular on-site meetings with referral sources. Maintains knowledge of entirety of PACE of Southwest Michigan’s offerings, programs and services and is able to educate referral sources accordingly, based on specified needs. Serves as referral source advocate by identifying needs and facilitates problem resolution. Responds quickly to potential leads and flexes schedule as appropriate. Maintains files and keeps database of referral sources’ staff names, preferences and marketing history, as well as daily marketing activity. Compiles monthly outcome report and continues to work for increased referrals.
Educates referral sources on PACE of Southwest Michigan capabilities, quality outcomes, comparisons to competitors, benefits and services. Maintains open and effective communication with the Executive Director. Cooperates with and supports PACE of Southwest Michigan intake and community outreach plan. Performs job duties in accordance with the mission and vision of PACE of Southwest Michigan.
Understands participant rights and incorporates them into daily practices. Meets PACE of Southwest Michigan standards for confidentiality. Performs other duties as assigned. Assists with intake calls to ensure prompt response to inquiries.
A MSW or RN with community outreach/marketing experience preferred. At least two years MSW or RN experience with community outreach/marketing healthcare service, experience preferred. Must have a m inimum of one year of experience working with the frail elderly population
