The Community Services Case Managers work within our In Home Therapy
program and/ or Community Support Program . Staff assist clients of
all ages who are considered to be at risk within the community.
Applicants who have prior experience with either residential
treatment or substance abuse treatment are preferred. The program
provides a home based case management service that encompasses the
client’s entire health profile, in order to reduce the number
of hospitalizations and improve clients’ ability to
function in the community.
For the In home Therapy
aspect, we are currently looking for clinicians to work as a team
with Masters level clinicians to provide therapeutic training and
support to children and their families. Community Support Program
case managers will work with individuals with mental health diagnosis
including substance abuse.
programs use a multi-disciplinary team approach focusing on
behavior management, skills training, along with an awareness of
community services and resources.
Candidates must have a High School diploma or GED and previous
experience with direct care.
