Responsibilities:
Thresholds, Illinois’ oldest and largest provider of mental health services, is seeking a Community Support Specialistfor our new Young Adult Community Support Team. This Community Support Specialist will have an exciting opportunity to be part of the creation of our multidisciplinary team, which will provide specialized services to young adults in their natural environments. The team will offer case management, counseling, advocacy, linkage and referrals, psychotherapy, psychiatry, collaboration with providers, employment coaching, and crisis intervention designed specifically for the unique needs of this population.

The Community Support Specialist will provide direct support services to the young adults within the program. This CSS must be or have been a recipient of mental health services. Furthermore, he or she must be willing to share those experiences with the young adults in the program .The CSS will be responsible for generating treatment plans, rendering case management and other supportive services, participating in clinical meetings and completing the required documentation. The Community Support Specialist works within a team-oriented, strengths-based approach, ensuring collaboration within the team and with other providers to facilitate integration of services.

Qualifications:

The successful candidatewill have experience providing services to young adults within the field of mental health. This person should have good written and verbal communication skills and strong basic computer skills, which include the ability to document services in a clear and effective manner. The CSS must function well independently and as part of a team. Must have a valid Driver’s license and access to a personal vehicle. Must have the ability to work alternatives schedules including evenings and weekends. Bachelors degree required, Master’s degree preferred and LPC, LSW, LCPC, LCSW a plus.
