Department for the Aging (DFTA) funded program for homebound seniors seeks a full time Community Worker Bilingual in Spanish/English to case manage and assess homebound seniors for DFTA services in their home.
Applicant will travel to client's home in Northern Queens (CD7) and meet with them and family to complete assessments and authorize services such as Meals on Wheels and DFTA funded Home care and Housekeeping. Worker will also be responsible for researching and referring for community services that are not funded by DFTA. Applicant will assist client in applying for various state and federal entitlements and benefits.
Applicant will be responsible for assessment, follow up and case recordings of various contacts with clients. Applicant will submit monthly statistics to supervisor and monitor time spent with clients.
Knowledge, Skills And Requirements
Applicant must hold a Bachelor degree. BSW or Bachelors in Social Service field a plus.
Bilingual Spanish/English a must.
Applicant must be able to travel to client's homes using public transportation or driving.
Applicant having experience with DFTA funded case management standards a plus.
Knowledge of entitlements for seniors a plus.
Applicant having experience with PDS data system and E-file system a plus.
Experience with Microsoft Word a must.
