§

Under the supervision of the Nursery Director, responsible and held accountable for all activities related to Patient Information, during their prescribed shift.

§

Reviews birth certificates for accuracy in spelling and data provided from the medical record.

§

Prints verification of birth facts at bedside and obtains either parent’s signature for verification of accurate information.

§

Ensures that all birth certificates are transmitted to the state within five (5) days of birth.

§

Maintains a birth certificate register.

§

Maintains Acknowledgement of Paternity (AOP) worksheets and ensures proper storage.

§

Responsible for entering birth certificate worksheets TER.

§

Prepares birth certificate for electronic signature from Manager/Director

.

§

Knowledge of Birth Certificate Requirements and laws.

§

Obtains statistical information required by the State.

§

Ensures maintenance of confidentiality of all protected health information.

§

Assists in clerical duties within the department as assigned by the Supervisor/Manager.

§

Identifies and assists with opportunities for performance improvement activities in the department and with other service areas.

§

Demonstrates fiscal responsibility by taking ownership of time management, decreasing waste (of time and supplies) and promoting cost effectiveness while maintaining patient safety and quality of care. To actively market services to increase volume and awareness.

§

Demonstrates the ability to be an advocate for and strive to protect the health, safety and rights of the patient.

§

Interactions with patients and colleagues demonstrate compassion and respect for the inherent dignity, worth, and uniqueness of every individual, unrestricted by considerations of social or economic status or cultural beliefs.

§

Communicate effectively with other care providers (physicians, nurses, business office, and medical records) to provide for continuity of patient care and to meet the needs of the customers we serve.
