The Coordinator for the Asian American Diabetes Initiative (AADI) provides case management to Asian patients at the Joslin Diabetes Clinic.
S/he manages internal and external AADI projects, and develops strategic programs and relationships with community partners.

Responsibilities:
Functions as a patient liaison.
Coordinate and manage medical visits for Asian Clinic patients and families. Communicate regularly with families, primary care hospital staff, insurance providers, pharmacies, community centers, and Joslin Clinic with the purpose of providing case management to patients and families.

This role will involve the following:
--Answer, filter, and log all AADI/Asian Clinic direct line and emails. Consult with AADI on action plan and ensure completion of all items discussed.

--Maintain an accurate, organized and up-to-date Asian Clinic database of all patient appointments, medical data, questionnaire data, and feedback. Generate customized reports as-needed for quality improvement and research purposes.

--Organize weekly Asian Clinic team meeting with notes and feedback regarding specific patient and family questions and interventions. Ensure completion of all items discussed.

--Assist AADI/Asian Clinic research and programs, including clinical studies, personalized patient folders, and group education visits.

--Function as an AADI liaison to the patient community.

Qualifications:
Bachelor’s Degree
1-2 years office experience, health care setting preferred.
Ability to speak Cantonese and/or Mandarin highly preferred in order to converse with the majority of the current Asian Clinic patient population.
Strong interpersonal skills. Be culturally sensitive to a diverse population of patients and staff.
Excellent written and verbal language skills.
Ability to prioritize and complete tasks in a timely manner when there are pressures of deadlines.
Ability to consistently perform detailed work to ensure accuracy in collecting and recording data.
Ability to engage with community partners to develop strategic relationships.
Demonstrated ability with PC and various software applications including MS office software, eg MS Word, Excel, and Powerpoint. Data base management a plus.
Experience working with diverse (ethnicities, languages, age, etc) population in community setting a plus.
