This is a 1 - Full-time first shift, 3 - Part-time second shift, 1 Periodic third shift position.

Location Charlottesville, VA

Qualifications Bachelor degree and one year service experience with the population required
Direct support experience with both developmental disabilities and mental health populations preferred
Experience with crisis prevention and intervention services preferred
Maintain valid driver license and insurance for personal vehicles potentially utilized for work. Complete within specified time period the trainings as required by agency and service model

Responsibilities The Crisis Therapeutic Home Counselor provides support to persons with co-occurring developmental disabilities and mental or behavioral health needs who are temporarily accessing crisis or planned respite within the REACH Therapeutic home
This staff participates in the daily routine of the home and provides support for habilitative activities, personal care, health care and other activities
Counselors participate as part of comprehensive REACH services that collaborate with individual’s primary provider and clinical home, community crisis services, family and others
Assists individuals with daily activities of personal care, behavioral health needs as well as health, wellness and leisure activities
Establishes rapport with individuals experiencing challenging behavior issues while maintaining respect for each person’s dignity, individuality and right to confidentiality
Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports in accordance with crisis guidelines and person’s plan. Plans, structures and prompts social and leisure time activities, including activities that build skills for behavior management, stress management, communication, adaptive living and daily living
Maintains timely documentation, progress notes, and other data, including incident and accident reports
Responds to individual safety needs, including reporting significant changes in behavior and health
Communicates with other staff and supervisor on progress, needs, concerns and changes
Performs housekeeping duties as assigned, including meal preparation in accordance with individual dietary needs and preferences
Adheres to program standards and policies as well as local, state and federal regulations
