Facilitates the growth and community interaction of an individual with neurological impairments, learning disabilities, or autism. Provides a variety of services necessary to enable the individual to live successfully in the community while providing relief to individuals who provide primary care and support. Works one-on-one in the consumer's home and community to address a variety of life skills to include social, recreational and other activities of daily living.

Positions are listed below by County: Albany County Openings: ALBANY- We are looking for an experienced direct support professionalto work one on one with a 21 year old man who lives with his mother inAlbany. Hours would be Tuesday 1:00-5:00 minimally with flexibilityfor more time as needed throughout the week. He wants support tomaintain and expand his current community inclusion activities,including swimming, participating on the Special Olympics track team,volunteering, walking, food shopping, and continuing to developeffective communication skillsALBANY- We are looking for a DSP for Community Habilitation to workwith a 27 year old woman who lives with her mother in Albany. We arelooking for someone to support her approximately 10 hours per week,2 to 3 half days (either mornings or afternoons), Monday –Friday.Skill development includes organization, planning, communication, andsafety. She is quite social and eager to learn.DELMAR- A 16 year old living with his parents and twin sister wouldlike a DSP for Community Habilitation on Wednesday afternoons. He enjoys being a part ofhis community and getting to know people he sees regularly.MALTA- We are looking for a DSP to work 2 times each week, for 3hours each session with an 8 year old boy with a developmentaldisability who lives with his parents in Malta. He is very active,loves swimming at the Y, walks, bowling, and the library. He requiressignificant support in areas of self care, following directions, andsafety, both at home and in his community.Schenectady County Openings-Guilderland Area Schenectady- A male in his 30’s, living with hisfamily, is looking to go out a few afternoons each weekday from about11:00-2:00. He would like support with exercising, healthy dining,shopping, walking. He would like assistance with decision making,assertiveness, increased independence and money skills.

A 33year old woman with a developmental disability who lives in Colonie with her family is looking for support one evening a week, for 4 hours each session. She enjoys cooking, baking, crafts, and accessing her community for music, shopping, movies, and outdoor events.

A 26year old man with a developmental disability who lives in Albany with his mother is looking for support to access his community one evening each week for 4 hours. He wants support to stay out of trouble by following social and program rules and guidelines; support to decrease his anxiety; and support to vary his routine. He enjoys shopping at local malls, eating out, history and the library.

COHOES- An 18 year old male living with his girlfriend and newborn would like a community hab staff to support him with obtaining his GED, volunteering in the community, understanding the consequences of his behavior, and skills to live independently and successfully as a family in their home. He enjoys video games, bicycling, and sports.

Columbia County Opening:
KINDERHOOK- Part time Community Habilitation DSP needed to support a
25 year old woman who lives with her family in Kinderhook. We are

He wants to stay healthy through healthy eating and exercise. He enjoys swimming, bowling, going to the book store and library, and going to Dunkin Donuts. He is open to trying new and varied activities which will expand his world and enrich his life.
CLIFTON PARK- An older teen boy living with his family would like an
additional worker on M or W after 3:00 to assist him with

in the community. He enjoys going to the Y, simple cooking, dining
out, going to the library, sports and playgrounds.

Qualifications

High school diploma or general education degree (GED). Medication Administration based on program need, First-Aid and CPR certification or eligibility for certification. SCIP/PROMOTE certification based on program need. Daily access to an automobile is required. A valid New York State Driver's License and ability to drive and driving record acceptable for agency insurance coverage are required. CPR and First Aid certification or eligibility for certification. Applicants hired after April 1, 2005 must be cleared and maintain acceptable record under the NYS OPWDD mandated criminal background check process.
