DESCRIPTION

Located at Mojave, CA

The Case Manager is responsible for coordinating services for the program's target population and their families as required by the Scope of Work or other contract requirements. The Case Manager will provide supportive counseling to clients and families, act as a liaison between the client and service providers, provide information and referrals to appropriate resources, and perform advocacy services. It is the overall task of the Case Manager to coordinate the client's comprehensive health, nutritional, educational, psychosocial, job training, and related support services and to educate clients on becoming independent consumers of the service delivery system.

DUTIES

Essential Functions:
Responsible for conducting an intake assessment on each new client according to program requirements.
Responsible for completing comprehensive assessments in the areas of health, nutrition, psychosocial needs, education/vocation, goals, parenting skills, and infant/home safety for each client enrolled in program.
Responsible for developing and implementing individual service plans.
Advocates for clients when there is a problem in the service delivery system.
Provides guidance counseling and support to clients and families.
Responsible for maintaining assigned case load and client contacts as required by Scope of Work, contract requirements and/or program protocols.
Serves as liaison with other community agencies and schools.
Develops and implements support and educational groups.

QUALIFICATIONS

Qualifications:
Prior experience working with at-risk children and families that are referred by Child Protective Services.

AA or AS in a health and human services..
Experience working with target population desirable
Ability to work effectively in an interdisciplinary team approach with both paraprofessionals and professional.
Ability to maintain patient dignity and confidentiality.
Bilingual (Spanish-English) preferred
Willingness to work non-regular hours as required.
Committed to the belief in health care with dignity for all, and that patients/clients have the right to health care information and participation in planning their own health care.
Must have a California drivers license, auto insurance in your name and clean DMV record.
