TITLE: DIRECTOR - LONG TERM CARE ACCESS
DEFINITION : Under the supervision of the Director for Long Term Care this position is responsible for the day to day operation of the Long Term Care Access Department. This responsibility includes the assessment of individuals applying for both institutional and community based long term care services, as well as the special SSI residential supplement and enrollment into the Aging Waiver program. It also includes the screening of nursing home applicants under the OBRA ’87 PASRR requirements and oversight of PCA’s Community-based Care Transitions Programs (CCTP).
MSN, MSW or related graduate degree.
Four (4) years supervisory experience and six (6) years of direct casework or clinical experience, preferably in the area of public health, discharge planning, aging services, or long term care.
DUTIES AND FUNCTIONS :
a. Oversees the LTC Access Department, with emphasis on assessment and level and locus of care determination, and the CCTP programs.
b. Supervises and directs the performance of the LTC Access Staff with direct supervision of the Nurse Supervisor and Assistant Director staff.
c. Implements State and federal standards and regulations applicable to the assessment of Long Term Care applicants, the Pre-Admission Assessment program, OBRA, enrollment of Aging Waiver applicants, and CCTP, and assures quality provision of service.
d. Assists in developing the departmental service and program objectives based on service demand and utilization, assists in developing, updating and implementing operation procedures for the LTC Access Department and implements procedures.
e. Develops and institutes necessary programmatic instruments including automated data processing and client information systems.
f. Oversees the recruitment and hiring of program staff and approves and implements staff training.
g. Identifies the Program’s promotional outreach and P/R needs and provides training, along with departmental Administrative team to major users and referral agencies. Oversees and approves reports and statistical requirements of the Department including CCTP
h. Prepares the annual budget and service projections and completes all approved budget revisions.
i. Oversees and monitors a quality assurance program consistent with the agency’s quality assurance objectives.
j. Oversees departmental consultant contracts and reconciles and approves invoices.
k. Serves as contact person for the Department of Public Welfare’s regional staff for OBRA cases and issues.
l. Serves as contact person for the Independent Enrollment Broker (IEB) and Aging Waiver Service Coordination entities.
m. Attends regional AAA meetings with PDA staff and attends other Departmental meetings as required and convenes regular meetings of the LTC Access staff.
Interfaces with other LTC Department Staff, CCTP hospital partners and the Centers for Medicare and Medicaid Services (CMS) personnel and participates in administrative functions of the LTC system as appropriate.
Serves as PCA’s Voter Registration Liaison to the Department of State.
Assures a strengths-based, consumer centered approach in program and services.
Performs other duties as required.
STARTING SALARY : $66,154.00
