POSITION: Resource Manager/Discharge Planner
CLASSIFICATION: Non-Exempt
REPORTS TO: Director of Patient Care Management

I. POSITION SCOPE

BASIC FUNCTION: To provide resource management as a part of comprehensive discharge planning and implementation. Responsible for assessing, planning and providing services to meet the social and emotional needs of patients and their families so that medical needs can be addressed adequately.

The Resource Manager/Discharge Planner collaborates with the physician, health care disciplines and the patient and/or family to identify and assist the patient to move from one level of care to another including pre and post hospital needs and services. The Resource Manager/Discharge Planner assesses plans, directs and evaluates an effective discharge planning process.

II. POSITION ACCOUNTABILITY

The Resource Manager/Discharge Planner is accountable to the Director of Patient Care Management, physicians, department directors, administrator on call, patients and families.

III. HOSPITAL-WIDE ACCOUNTABILITIES

1. Performs job duties in a manner that supports the values.

4. Adheres to hospital policies supporting patient rights and confidentiality.
5. Participates in development and attainment of departmental and/or WCCH annual Business Plan Goals.

IV. SIGNIFICANT RESPONSIBILITIES, ACCOUNTABILITIES, and ADMINISTRATIVE RESPONSIBILITIES

1. Responsible for assessment and evaluation of psychosocial needs upon
referral of: patients, their families and/or significant others. To provide crisis intervention, when patients, and/or families safety is at risk.
2. Conducts on-going evaluation/research of community resources as it
relates to the agency and its patients, and keeping current on changes made within community resources realm.
3. Develops and implements a strong discharge plan to meet the patients' needs. To assist/arrange patients; families; with extended placement when
necessary. To professionally document direct and indirect patient contact, as set
forth by policies and procedures by the agency, CMS, and other payers.
4. Assessment of financial needs of patients in relation to meeting medical
needs and participates in the development of effective resolutions that will benefit the patients well being and safety.
5. Participates in continuing education and other meetings.
6. Assists the physician, agency and other health care team members in
understanding the significant social and emotional factors as they relate to the health and safety of the patient.
7. To assist in the interpretation of social resources to agency staff to
increase knowledge of psychosocial factors and how all these impact
patient and/or the family system.
8. Functions as an Interdisciplinary Team member.
9. Participates in department's performance improvement program/utilization
review of Patient Care Management

V. QUALIFICATIONS

Preferred: Associates Degree in Human Services or related field OR Bachelor's Degree in Human Services, Developmental Management, Social Sciences, or related field

Preferred: Associates Degree in Human Services or related field OR Bachelor's Degree in Human Services, Developmental Management, Social Sciences, or related field.

B. Skills
Excellent written and verbal communication skills. Good analytical skills contributing to productive decision making and effective utilization of resources. Working knowledge of local, state, and federal resources as well as application processes and criteria mandates for financial assistance programs. An extensive knowledge of Virginia Department of Social Service mandates and state guidelines preferred. Extensive knowledge of community offerings, referral processes, and must demonstrate the ability to work independently, creating an efficient and productive environment. Must have strong computer skills.

VI. PHYSICAL DEMANDS

The physical demands of this position include sitting 20% of work time, standing 30% of work time, walking 50%- 80% of work time. The majority of time is spent walking in clinical areas, face to face encounters and meetings with patients and families. Works with or has access to confidential material. Works with patients of all ages. Duties vary; uses gross and fine motor skills, may occasionally lift and position patients. Pushes, pulls and lifts equipment and supplies. May be required to lift 75-100 pounds, carry 50-75 pounds, push 200-250 pounds. Temperature controlled, well-lighted, well-ventilated controlled environment. There is a potential for exposure to hazardous materials as defined in MSDS manual; chemotherapy drugs, exposure to x-ray equipment, and blood and body fluids through patient contact. May use basic office equipment.

VII. PERFORMANCE EVALUATION CRITERIA

In addition to the criteria listed above, all employees are expected to recognize and respond positively to hospital patients, physicians, co-workers and visitors. Caring for people is the primary mission of Wythe County Community Hospital and requires the support of all employees.
A. Education/Experience
