Job Description:
Job Summary:
Responsible for activities associated with reimbursement & discharge planning. This role coordinates the discharge planning needs of client hospital’s patients using standards of care, policies and procedures. In addition to the facilitation of these processes, this position also collaborates with the patient Nursing Home, Mental Health, and Substance abuse Facility regarding target length of stays, payer requirements and care options available to meet patient needs. Maintains extensive resource knowledge regarding all social & community services while demonstrating consistent ability to work the hospital, patient, and families to incorporate appropriate resources and facilitate timely discharge with quality clinical outcomes.

Major Responsibilities:
Mobilizes resources and manages healthcare system in response to individual patient needs according to the hospital client’s interdisciplinary plan of care.

Review assessment of patient's anticipated discharge needs and implements discharge plans in a timely manner in accordance with hospital's policies.

Interacts with health care providers to facilitate movement of the patient through the system.

Works with Hospital and other disciplines to insure patients receive appropriate and timely education for discharge.

Serves as a change agent and supports the strategic direction of hospital by identifying barriers to patient throughput and communicates/advises possible solutions to appropriate leadership.

Utilizes active collaborative participation with Hospital Staff in multidisciplinary rounding, case conferences & discharge planning meetings to advance plan of care.

Initiates and maintains collegial relationships with all members of the Hospital discharge team, including but not limited to: external agencies (skilled nursing facilities, home health agencies & their liaisons, etc) staff of other agencies such Department of Social Services, Department of Immigration Naturalization Services, and third party payers/insurance.

To facilitate timely discharges/transfers, seeks early identification of the appropriate reimbursement source for nursing home placement or rehabilitative services. Request Prucol negotiations with INS to facilitate the Medicaid application process for undocumented patients. When the patient lacks the mental capacity work with Elder Law Department in initiating a guardianship.

Investigates and recommends alterative post-discharge delivery sites (home health care, skilled nurses, rehab, etc), orchestrates care between hospital and facility.

Works effectively with hospital staff, patients, and families to reduce the incidence of avoidable days and to lower the hospital's overall average length of stay.

Effectively communicates verbal and written with client hospital to help implement an appropriate discharge plan to meet a patient's needs.

Documents anticipated discharge plan in medical record and updates plan as needed.

Attends in-services and is compliant with essential education without prompting from manager.

Performs other duties as assigned.

The Discharge Planning Coordinator is required to have:
Ability to work effectively in a self-directed role.

Must possess excellent written and verbal communication skills, as well as interpersonal and team-building skills.

Understanding of Healthcare System and Nursing, Mental Health, and Substance Abuse facilities.

Knowledge of Social and Community Services available.

Can do attitude and use all resources at their disposal for an effective outcome.

Leadership experiences a plus.

Skilled in using Outlook, Word, and Excel with the ability to learn internal databases.

Schedule:
Monday through Friday, 9:00 a.m. - 6:00 p.m., Salary 40-45k per year.
