Company Information: Youth Advocate Programs, Inc. (YAP) is one of the largest non-profit Youth and Family Support agencies working with high-risk youth and their families in the United States. Currently, YAP employs more than 2,000 plus dedicated workers and serves approximately 12,000 families per year. Our mission is to engage human service systems so that they rely less on institutional care and invest more in supporting families and neighborhoods. We currently work with child welfare, juvenile justice, behavioral health, disability, and education systems to develop and offer community-based alternatives for the highest risk children, young people, young adults and families.
Summary of the Position: The Intensive Family Coordinator will provide a professional level of case management services as well as direct advocacy services to a caseload of families. The Intensive Family Coordinator will be responsible for completing family assessments and creating individualized service plans. The Intensive Family Coordinator will be responsible for providing services for truancy program in the school district. The Intensive Family Coordinator will report to the Director of the Program. Successful applicants will possess outstanding people skills, creativity and energy as well as expertise of the Bronx and its resources. Applicants will be versed in truancy, school issues, and assessing family dynamics and issues. YAP, Inc. is a non-profit organization committed to strength-based, family-centered programming as a means of keeping families fully intact, and youth successful in their homes. YAP, Inc. relies heavily on the use of informal supports drawn from resources found in the home neighborhoods of client families to build a network of support for each youth and family referred. Travel will be necessary as well as willingness to work nights and weekends and provide 24/7 crisis support to the caseload.
Qualifications/Requirements: The position requires a Bachelor's degree in Human Service type field or 2 years relevant experience. Position requires ability to travel through South Bronx community, and larger NYC community. This position requires background checks to be completed. Bilingual (Spanish/English) applicants preferred.
