Liaison with primary care practices to coordinate preventative and chronic care of patients. This position will work with a team and clinic involved in leading the change from traditional healthcare to preventative health maintenance.
Identify care gaps and proactively conduct/refer patients for preventative screenings.
Work collaboratively with the healthcare team to adhere to the patient's individualized plan of care.
Coordinate referrals and other community resources.
Coordinate patient appointments with healthcare team, maximizing clinic efficiencies.
Work with the healthcare team to continuously evaluate workflow processes, patient access, and care coordination.
High school diploma or GED required.
Minimum 3 years of nursing or medical assistant experience required.
Unrestricted LPN license by the State of Colorado Board of Nursing or compact privileges under the multi-state Nurse Licensure Compact agreement. OR Active Registered Medical Assistant (RMA) certification and proof of current membership from American Medical Technologist (AMT), OR Active Certified Medical Assistant (CMA) certification from American Association of Medical Assistants (AAMA) or the National Center for Competency Testing (CNCCT).
Active American Heart Association BLS for Healthcare Providers.
Medical Assistance leadership experience.
Internal or Family Medicine experience.
Excel spreadsheet or EHR experience.
