This is a PRN position.
High School Diploma or equivalent. Two years’ experience working with individuals with severe and persistent mental illness and dual diagnosis is preferred; two years working in group home setting or assisting adults diagnosed with AMI with activities of daily living preferred.
Assists individuals with activities of daily activities, home care and maintenance, and transportation. Participate in implementation of person centered plan. Provide direct interventions to build socialization, behavior management skills and daily/community living skills. Maintain documentation, progress notes, and other data as needed, including incident and accident reports. Communicate with other staff and supervisor on progress, needs, and changes. Respond to individual safety needs, including reporting significant changes in behavior and health. Provide high quality customer service, serving as a positive representative for the agency with other professionals, families, and the community large. Perform housekeeping duties as assigned. Must be able to bend, and lift at least 50 lbs. Must have valid NCDL.
