Job Description:
MUST BE A REGISTERED NURSE

The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required. To provide a smooth transition from the acute care and skilled nursing facilities to Advocate Home Health Services’ entities for those patients requiring home care. The Home Care Liaison works in conjunction with discharge planning staff to assist the patient/family in the initiation of home care services. Completes final assessment on complex referrals involving infusion therapies, complex wounds and/or enterals. Accountabilities: A. Promote continuity of care by facilitating appropriate and timely referrals of designated patients to Advocate Home Care. Upon referral to Advocate: 1. Interviews patients/families in the hospital, to assess and evaluate patient for home care appropriateness. 2. As part of the initial referral evaluation, verifies home health care benefits according to payor plans. 3. Ensures that doctors initial home care orders are obtained and assures follow-up. 4. Documents initial plan of home health care and initiates interdisciplinary communication. 5. Completes predischarge assessments/forms for high tech patients to be followed by home care. 6. Establishes and maintains a system of communication specific to patients and the ir receipt of home care services. 7. Redirect requests received from physicians, hospital staff or family members for home care services to the facilities discharge planning staff for assessment. B. Promote collaboration and coordination between the hospital and Home Health Company to achieve continuity of care and utilize health care resources more effectively. Coordinator recognizes the needs and behaviors of various age groups. 1. Act as resource to patients/families and hospital and skilled nursing facility staff (i.e. social work, nursing) regarding types of services available and the criteria for home care, including Medicare guidelines. 2. Attends multidisciplinary and discharge planning meetings that involve Advocate Home Health patients/issues. 3. Maintains a knowledge of the Medicare, Medicaid, and HMO procedures and guidelines. 4. Home Care Liaison-RN provides oversight/direction to Home Care Liaison-LPN. Will provide training to Home Care Liaison-LPN on HHC processes and policies. C. Responsible for keeping Manager informed of status at assigned facilities. 1. Assists in the development of policies and procedures for discharge planning for patients requiring home care, by making recommendations and reviewing proposed policies. 2. Assures appropriate follow-up to patient satisfaction issues relating to Advocate Home Health Services. 3. Generates weekly productivity reports that reflect home care, referral activity including type of services, payor mix and trends. This position will support hospitals in the North as well as CRD Intake in Oak Brook. Hours and days will vary and weekends will be involved. This is a REGISTRY position.
