HealthSouth's Home Health programs provide skilled nursing and therapeutic services, such as physical, occupational or speech therapy, medical social services, and other home medical arrangements.
Our team of Home Health professionals works closely with the patient's physician and family to develop a plan of treatment, tailored to meet the patient's individual healthcare needs. This team approach ensures consistent, coordinated care from professionals who understand the unique needs of homebound patients.
The Home Health Registered Nurseprovides skilled nursing and direct patient care according to hospital, Home Health, state, professional and federal regulations and guidelines.
In addition, the position:
Provides direct patient care to patients within their home
Requires supervising of care treatment and services of patient care provided by LPNs, RNTs, and other staff on assigned patient
Requires development, implementation, and evaluation of an individual plan of care for each patient
Requires thorough assessment and re-assessment of patients needs
Requires open and on-going communication with hospital departments to ensure patient, staff, and hospital needs are met
Requires the nurse to report patient concerns and changes in condition to supervisor and/or physician in a timely manner

You will be paired with mentors

License or Certification:
Current RN licensure appropriate to state regulations
CPR certification (ACLS preferred)
CRRN certification preferred
Valid Drivers License and acceptable driving record
Total Education, Vocational Training and Experience:
One year experience in Home Health setting preferred
