The Home Health RN provides quality care and coordinates home care to clients served by a Primary Care Team under the direction of the RN supervisor. The Home Health RN supervises and delegates to the Home Health LPN and Home Health Aide/CNA appropriately and coordinates with other team members, the physician, and clients.

Primary Responsibilities:
Coordinates with the RN supervisor to assure the client driven plan of care strives to meet positive outcomes through on going evaluation using evidences based practice.
Responsible for making certain the home care provided is compliant with State and Federal regulations (documentation and delivered) through participation in chart review and following established process.
Responsible for participation in orientation, on-going education programs and successful completion of competency requirements.
Responsible for providing services that are congruent with the policies of FHS within the scope of professional nursing practice as defined in the Nurse Practice Act.
Strives to provide services to meet both internal and external customer expectations.
Assumes individual responsibility for professional competence and professional nursing practice
Assures that the services provided are efficient and of value, meeting the expectations of quality healthcare delivery within operations goals of the Division, FHS and United.
Ensures home care is proved in accordance with agency and United Corporate Compliance Program.
Majority of job duties are carried out in patient's homes and driving throughout the community.
Some night visits when on-call.
Must be available to work a weekend day and do evening visits as necessary.

Requirements:
Current RN license in the State of Nevada required
High school diploma or GED required
CPR certification required
1+ years hands on nursing experience on a M/S unit
Willing to travel with in the Southwest region of Las Vegas for home visits
Must be able to work independently and has basic computer skills.
Ability to develop and implement plan of care based on assessment skills and demonstrates independent clinical and problem solving skills.
Good organizational verbal and written communications skills are required.
Valid Nevada Driver’s license and maintain personal auto insurance coverage and have reliable transportation
Demonstrates key technical skills utilized in the day-to-day management of home health patients.
Knowledge of professional standards of care and laws and regulations applicable to home health services. Technically proficient in field of practices.

Preferred Assets:
Home health experience a plus
Lab licensed a plus
IV certified a plus
Bachelor’s degree is preferred
Basic knowledge of home health requirements is preferred.
Skilled at using both windows and non-windows based software.
