The Home Health Supervisor provides direct patient care and administers medication and IVs and may be assigned to provide private duty or staff relief services. He/She reports to the Home Health Director, the Office Administrator, the Staffing Coordinator, and the Head Nurse of the facility and must work with patients, families, physicians, and other health care providers in a professional and caring manner.

The ideal candidate for this position is a graduate of an accredited school of Nursing, holds a current, unencumbered license as a Registered Nurse in this state, and can provide satisfactory references from previous and/or current employers. Home Health experience is preferred.

The following is a representative list of duties and responsibilities associated with this position:
-- Assesses and documents assigned patient's status on an ongoing basis
-- Develops and/or follows a written plan of care
-- Implements the care plan and documents the patient's response on appropriate forms
-- Notifies appropriate health care professionals of changes in patient's status (such as the Physician, Nurse Supervisor, or Home Health Director/Supervisor)
-- Encourages patient and family participation in the formulation and revision of the nursing care plan
-- Provides/Supervises direct patient care based on the policies and procedures of Care Advantage, Inc., and those of the Facility in which they are assigned
-- Demonstrates adequate knowledge and skill in regard to administration of medications (including IV infusions) and ordered treatments
-- Sets priorities and organizes assignments so as to complete them in a timely and efficient manner; seeks clarification from the appropriate health care professionals (such as the Physician and Nurse Supervisor) before implementing
-- Provides nursing care within the scope of practice based on knowledge, experience, and training as well as state licensure requirements
-- Demonstrates skill in oral and written communication, as well as interpersonal relationships with patients, families, and coworkers
-- Observes and reports symptoms and conditions of patients to the appropriate health care providers and the Home Health Director/Supervisor
-- Prepares progress notes and required nursing documentation accurately and completely
-- Assists the physician in performing specialized procedures, including preparation of equipment and supplies for treatments that required adherence to sterile or aseptic techniques
-- Supports the philosophy, purposes, policies, and procedures of Care Advantage, Inc.
-- Adheres to the dress code for Care Advantage, Inc.
-- Strives to develop and maintain team spirit
-- Extends courtesy and an attitude of cooperation when communicating with others
-- Shares knowledge and lends assistance to others when needed
-- Maintains confidentiality regarding patient information
-- Demonstrates responsible attitude towards attendance and promptness
-- Participates in in-service and training programs provided by Care Advantage, Inc., and other resources
-- Maintains current CPR certification according to the American Heart Association or American Red Cross standards
