The Clinical Manager, RN directs the provision of all Hospice Atlanta's services necessary to meet the hospice needs of patients assigned to the care team. This position is responsible for insuring the timely and comprehensive assessment of the patients needs; the development of individualized care management plans; implementation of the care plan through the delivery and coordination of services; and the monitoring and evaluation of all clinical outcomes to insure that the services are provided in a professional, comprehensive and cost-effective manner. Responsible for the periodic evaluation of the quality and integrity of all administrative and clinical practices. Makes routine on-site evaluations of all Clinical team staff. Acts as a Mentor for new clinical staff and insures their effective integration into the care team. Serves as a clinical resource person for the Clinical team personnel. Has ultimate responsibility for the assignment of duties, direction and evaluation of Clinical team and serves as a strong Leader for the Hospice Division.
Duties & Responsibilities:
•This is a "working manager" position
•Responsible for profit and loss, marketing / sales, customer relations / satisfaction, employee supervision, employee training / discipline, coordination of services.
•Supports corporate policies, goals and objectives.
•Supervises, directs and reviews activities of the interdisciplinary team.
•Hires, trains, supervises and terminates (with administrative approval) all interdisciplinary staff.
•Completes staff performance appraisals in a timely manner.
•Oversees Case Manager’s direction of patient care activities.
•Conducts interdisciplinary team meetings.
•Oversees the Case Manager’s development of the plan of care in accordance
with hospice philosophy, and the Charge Nurse's consultation with the medical director regarding attending physician orders and patient needs, as related to the plan of care.
•Supervises patient and family care as specified by the plan of care, assessing appropriateness, continuity and quality of care.
•Oversees to ensure that patient’s recertification’s are discussed & completed in a timely manner.
•Communicates patient status information to the business office.
•Promotes positive working relationship with patient, family, physician and other facilities.
Education & Experience
•Education: Graduate of an accredited Diploma, Associate Degree or Baccalaureate School of Nursing or Masters Degree in Social Work. Preference is given to individuals with Baccalaureate Degree or higher.
•Licensure/Certifications: Must have current RN licensure by the Georgia State Board of Nursing. Must have current CPR certificate.
•Previous Experience: 3 -5+ years experience in case management and/or managerial responsibility in a health care field. Must have at least 1-3 years experience in home health care or hospice/oncology care. Preference given to national certification in a clinical field.
•Knowledge/Training: Knowledge of Medicare, Medicaid and other funding source regulations as applied to home health care services. Demonstrate knowledge and competence in advanced practice nursing or social work skills, including collaborating with interdisciplinary team. Demonstrate knowledge of home health proficiency in quality of clinical records, productivity, customer satisfaction surveys and leadership role with staff. Must be able to function independently and effectively. Requires proven interpersonal, supervisory and management skills with ability to communicate effectively. Demonstrates self-initiative, proven decision making skills and time management skills. Ability to operate computer equipment, FAX and copier machines as well as maintaining communication with Company telecommunication systems.
•Transportation: Must have reliable transportation.
•Computer Skills: Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
