General Summary The Comprehensive Cerebral Palsy Program, Hospital-Based Team Service Coordinator is a Registered Nurse who will assist with the effort to improve overall outcomes in the patient’s health status. The Service Coordinator must be a flexible problem solver on multiple levels, from patient-level, to disease population, to care system. Service Coordination focuses on the broad range of services that are needed by the child with complex medical diagnoses / needs. It is a process that helps make sure that the family of a child with special health care needs finds the services he/she needs, is linked with the right providers, and has help getting care when care and services are either not available in the community or adjustments are needed with current services.

The team service coordinator will be a member of the CP Interdisciplinary Clinic Specialty Team and will collaborate with the patient’s family and BCMH Public Health Nurse, to ensure best outcomes. The Bureau for Children with Medical Handicaps (BCMH) sponsors this program. Minimum Requirements · Graduate of an approved School of Nursing; BSN preferred. If Associates must complete RN to BSN within 5 years of hire date.

  • Current Ohio licensure as an RN · Previous experience in a managed care environment strongly preferred. · Previous experience with Cerebral Palsy patients preferred; · Ability to read and interpret documents such as safety rules, and procedure manuals. · Ability to write reports and correspondence. · Must possess critical thinking skills · Must pass Pediatric Dosage Calculation test post offer and pre-hire at 100% accuracy · Must enjoy working in a fast paced environment with a diverse patient population.
  • Requires excellent proven customer service skills and the ability to function within a multi-disciplinary team Minimum Physical Requirements 1. Must be able to see, hear, stand, walk, speak, read and perform manual tasks with or without accommodation, and care for oneself with little or no difficulty. 2. Must be able to independently lift up to 25 lbs.
3. Must be able to move or reposition patients of any weight or size with the assistance of another person (s) and/or equipment.
