JOB TITLE: Intake Coordinator/Assistant Guardianship Coordinator
Reports to: Assistant Director of Medicaid Service Coordination
Hours/Days of Work : Full time, 37.5 hours per week, Monday - Friday. Additional evening and weekend hours as required.
Education/Experience: Bachelor's Degree; and one year of experience working with the physically, mentally or developmentally disabled; or a combination of education and specialized experience which, in the judgment of the Agency, is substantially equivalent to the standards of this subdivision.
Clean, valid New York State Driver's License.
Ability to take initiative, work independently, set priorities and meet deadlines.
Ability to adapt to change, while delivering successful and timely results.
Should possess exceptional skills in dealing with the public as well as solid management and organizational skills.
Ability to represent the Values and Mission of Liberty and the programs they provide to the community.
Knowledge of and proficiency in computer programs ie: Microsoft Word, Excel, Adobe, Microsoft Outlook, and experience with database management.
Ability to communicate effectively and serve as a liaison to primary referral sources, families, team members, school districts and community agencies.
MAJOR RESPONSIBLITIES -
The Intake Coordinator/ Assistant Guardianship Coordinator will act as the gateway to resources and supports regarding the services provided by Liberty ARC, as well as a resource of information about community programs for the individuals we support and individuals in the community. This position is directly responsible for receiving internal/external referrals, processing all referrals, screening applicants, and coordinating information and services with other agencies. The intake coordinator acts as the agency's liaison to schools and families to coordinate the needs of transitioning students. This position is responsible for the initiation, processing, record keeping and maintenance of all guardianships pending and approved applications of those individuals that we support. The Assistant Guardianship Coordinator also acts as the liaison to the NYSARC Guardianship committee.
Salary Range: 31,840-35,820 + our full benefits package!
