Must be either a licensed or board eligible/provisional licensed professional (LPC, LCAS, LPA, LMFT, or LCSW); Must have a minimum of one (1) year documented experience with population.
Must deliver services in various environments, such as homes, schools, detention centers and jails (state funds only), homeless shelters, street locations, etc.; responsible for coordinating the initial assessment and developing the youth’s Person Centered Plan (PCP); diffuse crises, evaluate nature of crises, and intervene to reduce the likelihood of a recurrence; ensure linkage to needed community services and resources; provide self-help and living skills training for youth; provide or coordinate (with another licensed professional) treatment for the youth or other family members; provide parenting skills training to help the family build skills for coping with the youth’s disorder; provide or arrange crisis management, case management, individual and/or family therapy, skills training, and other rehabilitative supports to prevent the need for an out-of-home, more restrictive services; provide “first responder” crisis response on a 24/7/365 basis to recipients; provide supervision to associate professionals and/or qualified professional team members; review staff service notes and ensure timeliness.
