To provide training and support for rehab consumers transitioning into competitive or supported employment, and to help placed consumers retain jobs long-term.
Ø Perform intensive on-the-job training with newly placed consumers.
Ø Train the consumers on public transportation alternatives to get to/from work.
Ø Provide ongoing coaching on a monthly basis for consumers needing long-term follow along (LTFA) after placement in order to help increase retention rates and problem solve any issues that may arise.
Ø Keep track of task analysis for the purpose of training new hires on the job.
Ø Completes all Job Coaching and LTFA case notes in a timely manner as per the case note compliance procedure. All case notes for the week need to be completed on Fridays by COB and forwarded to the appropriate Coordinator along with the weekly report.
Ø Track # of service hours provided to each consumer and submit activity reports on time to get vouchers for additional hours when needed.
Ø As a member of the Coaching Team, teach job search/job readiness skills workshops.
Ø Perform case management services as required, to include linking the trainee with other service providers or community resources such as: recreation, education, transportation or leisure, in order to further enhance the trainee’s integration into the community.
Ø Train consumers, new co-workers and supervisors on how to best work with the newly placed consumer.
Ø Educate consumers about how working may impact other benefits that they currently receive.
Ø Meet individual and team revenue goals.
Ø Contacts DVRS & other referral agencies for supported employment and/ or job coaching referrals.
Ø Submit monthly job coaching reports.
Ø Must become familiar with CARF regulations and assure that services provided and documentation maintained is CARF compliant.
Other regular responsibilities:
If the Job Coach is fluent in another language, may be asked to translate on behalf of rehab consumers as needed. Help prepare for and participate in CARF audits. Attend rehab dept. and all staff meetings.
Perform other duties and responsibilities as required by the JC/LTFA Coordinator.
Qualifications/Basic Job Requirements:
AA Degree in human services field from an accredited college. Alternatively, High School diploma and 3-5 years experience in related field. Knowledge of MS Word, Excel and Outlook required. For job coaches working with deaf and HOH consumers, fluency in American Sign Language required and SCPI certification preferred.
Must have a valid NJ driver’s license and pass license check. Must be willing to use private transportation to travel to employers and serve consumers in counties throughout Northern NJ
Special Working Conditions:
There are no unusual working conditions. Job coachis expected to work in the community in addition to at the main office. Conditions at employer locations may vary.
