Completion of an accredited course of Licensed Practical Nursing and must hold a license to practice in the State of Minnesota.
Must be able to lift 30 lbs.
Fluency in Spanish or Somali or Hmong or Vietnamese.

Preferred Qualifications:
Aptitude and/or interest in working with psychiatry patients in an outpatient mental health setting as part of a team.

Patient Contact/Care (Approximately 85%):
Interview and document patient's chief complaint (reason for visit).
Escort patients into exam room and prepare for provider's visit - to include BP, temperature, pulse, respiration, vision and hearing screening, and other assessments appropriate to visit type.
Assist provider with procedures, i.e., minor surgery, pelvic examination, ear wash, CPR, etc.
Perform wound care and apply dressings.
Administer injections and medications under the direction of a provider as per legal status.
Instruct patients per provider order, i.e., follow-up care, medications, and wound care.

Clinic Maintenance: (Approximately 10%)
Maintain exam rooms/replenish supplies/clean and sterilize instruments.
Perform lab procedures in the areas of: venipuncture, hematology, urinalysis, microbiology, pregnancy testing, electrocardiography, and spirometry.
Review charts and health histories for completeness, and respond to requests for results of diagnostic tests.

Care Coordination: (Approximately 5%)
Schedule appointments, review charts to plan care prior to clinic visit, follow up for clinic and specialty appointments, give lab results, and communicate with other members of the care team as appropriate.
Other duties as assigned.

Program/Unit Description

CUHCC's mission is to seek health equity in our community by advancing the well-being of diverse people. The clinic is known for providing high quality care to diverse patients and is moving forward in improving timely access to care for patients. CUHCC is building a care model that seeks to improve care at four different points: before the visit, during the visit, after the visit and between visits. This position will focus on providing care after the medical or psychiatry appointment has been completed. This position will work alongside the providers, interpreters, nurses and other members of the patient's care team. The initial focus of this position will be to serve those who are at high risk and who need immediate follow up upon completion of the visit to ensure that they have a path to the next steps in their care.
