As directed by the RN, assists with the monitoring and coordination of the delivery of health care services for individuals with developmental disabilities who are living in community based homes. Responsible for insuring confidentiality is strictly maintained concerning all agency related information.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES

•Assists with monitoring and coordinating the delivery of preventive and routine health care for individuals with developmental disabilities
•Adheres to and assists with ensuring facility compliance with all Physician, NP, PA, etc. orders.
•Assists with coordinating medical program needs with community services, medical providers and other related agencies.
•Assists with ensuring adequate scheduling and completion of health related exams, appointments, laboratory/diagnostic testing as ordered or required.
•Safely transports individuals to and from appointments if necessary.
•Assists with the monitoring of medication administration in the facility.
•Administers medications, treatments and supplements as assigned.
•Assists with providing health services training for staff.
•Participates as an active member of the Interdisciplinary Team.
•Maintains compliance with and assists with ensuring facility compliance with all applicable State/Federal regulations and agency policy and procedure.
•Maintains communication with Medical Liaison, Program Manager, Program Administrator, Service Coordinator and other staff as necessary.
•Complies with all agency policies and procedures
•Other duties as required.
