The LPN is primarily responsible for providing LPN nursing care for people with developmental disabilities. The LPN works under the direction of the RN.
Under the direction of the supervising site RN and the Director of Nursing, ensures that all medical and related needs of clients are met as directed through:
Implementation of the medication and treatment administration process.
Implementation of nursing care plans.
Accurate nursing documentation with a minimum of weekly nursing notes on each client.
Maintain continuous observation of client health needs and report any changes/concerns immediately to the RN followed by appropriate written documentation.
Provides hands on treatment/nursing procedures.
Implements nursing procedures as directed.
Implementation of physician, dentist, and nursing orders.
Documenting any changes in status, concerns, or information from consultants and MD.
Assist in maintenance of the Medical Administration Record for each client.
Relate to clients in a continuous, consistent, warm, caring, age-appropriate manner.
Carry out other duties as assigned.
Participate in nursing staff meetings and in-service training opportunities.
Participate in plan of care.
Be knowledgeable regarding all Nursing policies, procedures, and statement of purpose of the program.
Maintain a safe working environment, be knowledgeable of and participate in fire drills. Use safe lifting techniques at all times.
Other duties as assigned by the Nursing Services Program Director.
Current license as a Licensed Practical Nurse in the state of Oregon, with no restrictions
One to four years nursing experience
Ability to obtain a CPR and First Aid Card
Valid driver’s license with a driving record that permits coverage under the agency’s corporate auto liability policy
Personal automobile liability insurance
Prior medically fragile and MR/DD experience is preferred
