The Licensed Practical Nurse (LPN) for the Oncology Department provides patient care under the supervision of the Adherence Nurse Manager. The Licensed Practical Nurse performs acts which require substantial knowledge, judgment and nursing skill based upon principles of psychological, biological, physical and social sciences. This position is responsible for making decisions based on his/her education preparation and experience in nursing.
This position reports directly to the Adherence Nurse Manager.
There are no direct reports to this position.
Education : Completion of the Licensed Practical Nursing certification required.
Experience : One year of current nursing experience. Hospital or Disease Management experience preferred.
Licensing/Certification : Valid license to practice as a Licensed Practical Nurse in the State of Michigan. NAPNES Certified.
Knowledge of Adherence and Drug Therapy Management policies and procedures.
Ability to recognize needs of and express concern for patients, families, caregivers, community organizations, providers, and clients.
Must be detail-oriented and be able to multi-task.
Strong phone and computer skills required.
Ability to maintain constructive working relationships with patients, families, co-workers, community organizations, and physicians.
Be able to work well with various personalities and work as a team.
Should be organized and able to prioritize and manage time effectively.
Must be dependable and able to act with discretion maintaining levels of strict confidentiality.
General knowledge of Internet navigation and research, e-mail, fax transmission, and copy equipment are essential.
Physical : Must have good vision and dexterity of hands and fingers. Must be able to stand, walk, crouch, stoop, bend, climb, kneel, keyboard. Must be able to lift up to 35 pounds above head. Must practice safe body mechanics.
Mental : Basic math skills and computer aptitude.
Language : Fluency in English with outgoing personality that translates to phone skills with excellent verbal and written communication skills.
Reasoning : Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
Technology : Proficient in Windows based operating systems and Microsoft Office programs including Outlook, Word, and Excel. Basic Rx30 software and other computer based programs.
Variable in climate controlled office environment.
DUTIES & RESPONSIBILITIES
Plan patient care based on assessment data and nursing diagnosis.
Implement the patient care program.
Evaluate patient care.
Interact closely as a patient liaison concerning initial and existing patient consultations.
Must be knowledgeable of drug side effects and interactions.
Correlate between drug manufacturers, physicians and staff to provide seamless prescription process.
Cross train when needed.
Verify information; give payment options, set up refills and deliveries.
Triage of orders for patient counseling.
Administer IV medication when needed.
Other Duties and Responsibilities as assigned.
