The LVN II works under the direct supervision of a Registered Nurse to provide for the care, treatment, and general welfare of patients. Responsibilities include: medication administration, noting and transcribing physician’s orders, assisting with sick call, providing/monitoring treatments and procedures, performs screening tests as ordered, collects specimens for laboratory examination, scheduling necessary appointments/consultations, maintains accountability for controlled medications and the security of assigned keys, Implements established patient/family education plans, provides individual or group teaching, for patients with specific health maintenance goals, prepares nursing and medical summaries, participates in the provision of appropriate psychiatric rehabilitation, using the Social Learning approach to treatment, attends change of shift report and communicates relevant information to support continuity of care. May receive collateral, consultative coaching and/or training from Social Learning Implementation leaders or consultants. Is knowledgeable of individual treatment plans and actively seeks ways to assist patients to meet their goals. Communicates vital information to the treatment team in relation to progress or lack thereof. Serves as a preceptor/mentor for LVN students and new nurses. In the absence of the RN, may assume responsibility for patient care and management of the unit, within their scope of practice, with RN support via telephone and immediate onsite availability as needed.

Completion of work requires moderate supervision with limited latitude for the use of initiative and independent judgment. The LVN has no financial responsibility but is accountable for assigned property.

Provides nursing support as assigned by the RN with the understanding that duties, location of work, and shift patterns are subject to change as determined by administration to meet the needs of the hospital.

Essential Job Functions:
EJF 1. Prepares and administers medications according to acceptable standards, hospital policies and procedures and within the parameters of their educational preparation.

EJF 2. Provides for the care, treatment, and general welfare of patients to include assisting with sick call and medical emergencies, providing/monitoring treatments and procedures, performing screening tests as ordered, collecting specimens for laboratory examination, scheduling appointments/consultations etc. % Time Spent:

EJF 3. Ensures comprehensive and accurate documentation to support the following: medication administration, treatments, changes in mental/physical condition, education provided, etc. according to MHRS and CWS guidelines.

EJF 4. Reports significant observations in mental/physical condition to the supervising RN or other team members as appropriate to promote quality patient care. % Time Spent:

EJF 5. Maintains accountability for controlled medications and the security of assigned unit keys.

EJF 6. Implements established plans for patient/family education, provides individual or group teaching as appropriate to increases patient to promote knowledge of medications, health/hygiene skills, disease management/prevention etc. to promote independent living skills. % Time Spent:

EJF 7. Participates in the provision of appropriate psychiatric rehabilitation, using the Social Learning approach to treatment, attends change of shift report and communicates relevant information to support continuity of care. Is knowledgeable of individual treatment plans and actively seeks ways to assist patients to meet their goals. Communicates vital information to the treatment team in relation to progress or lack thereof.

EJF 8. Demonstrates a commitment to a culture that minimizes events that lead to restrictive interventions through an understanding and application of the following concepts: a) Primary Prevention, b) Secondary Prevention, and c)Tertiary Prevention % Time Spent:
EJF 9. Maintains a medication/treatment area that is clean, neat and orderly. Responsibilities include wiping down counter tops and medication carts at the end of each shift, seeing that all containers are properly labeled and stored in an acceptable location, and ensuring stock medications and treatment supplies are available and within date for safe use.

EJF 10. Serves as a preceptor and mentor for LVN students and newly hired nurses, using the program specific orientation guidelines to promote consistency within the program, and encourage retention.

EJF 11. Assumes responsibility for patient care and management of the unit with RN support via telephone, and immediate on-site availability as needed.

Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.

1. Use of Medical Equipment to include: glucometer, AED, Oxygen
2. Demonstrate skill and capability of accurately observing and identifying significant patient behaviors and possesses the capability of accurately describing such events in the patient’s record
3. Psychiatric Rehabilitation Social Learning Approach to Treatment
4. Group Process to include: medication education, disease processes, disease management/prevention

Registration or Licensure Requirements:
Licensed as a LVN in the state of Texas or in a party state that recognizes reciprocity through the Nurse Licensure Compact as described in Art. 4528b of the Nurse Practice Act.

Initial Selection Criteria:
