This position is responsible for overseeing social workers as they work to expedite the patient’s use of services along a continuum of care and restore and/or maintain the patient’s independent functioning to the fullest extent possible.
Responsible for training and mentoring newly hired social workers. This position is responsible for keeping the social workers up to date with current services and processed in the community.
Acts as a counselor and liaison between patient, family, physician, healthcare providers, and community resources. Assists in understanding normal responses to illness and resulting lifestyle changes. Actively participates in the collaborative program which requires review of patient activities related to medical necessity of hospitalization, appropriateness of length of stay, timely and appropriate utilization of services rendered and identification of patient needs related to patient care following discharge. The position promotes helping relationships with the patient and family, quality care and cost-effective outcomes that enhance the physical, psychosocial, and vocational health. It includes assessing, planning, implementing, coordinating and evaluation health-related service options throughout the continuum.
Each patient is assessed by appropriate disciplines beginning at the time the patient enters a hospital service and progressing through discharge. The patient’s relevant physical, psychological and social status will be assessed. The designated ages of the patient population are:
Infancy Birth to 1 year
Toddlers Age 1 to 3
Preschool Age 3 to Age 5
School Child Age 5 to Age 12
Adolescent Age 12 to Age 18
Young Adult Age 18 to Age 44
Middle Age Age 45 to Age 65
Older Adult Age 65 and Over
1. Current licensure in Texas as a Social
2. Two years experience as a Licensed
Social Worker in acute care, desirable.
