This position is located in Serra Mesa, area of San Diego CA.
8-hour shift: 8:00 am – 4:30 pm, on-call weekends as needed.
Required Skills and Qualifications
- Master's in Social Work.
- Experienced and/or willing to obtain certification in Palliative Care and Oncology Social Work Practice.
- Minimum five years of acute care/hospital setting experience.
- Assessment, Counseling, Case Management, Resources, and computer skills.
- Experience with providing psychosocial assessment/ clinical counseling services and crisis intervention to patients/families in an acute care hospital setting.
Preferred Skills and Qualifications
- California Licensed Clinical Social Worker (LCSW) preferred or board eligible
- Bilingual (English/Spanish)
Medical Social Work coverage 40 hour position covering Oncology. The Social Worker should have strong clinical skills and knowledge of current treatment options. Provide psychosocial assessment/ clinical counseling services and crisis intervention to patients/families in an acute care hospital setting. Work with members of an interdisciplinary team to develop plans of care that support patient's adjustment to illness and hospitalization. Collaborate with RN Case Management the development of a safe discharge plan and facilitate for unfunded patients referrals to community resources for continuity of care. Screen patients for depression, anxiety, substance abuse and acute psychiatric illness as it impacts hospital care, and future health care access. Assess for psychosocial risk factors e.g. domestic violence, child abuse, and elder abuse/neglect, homelessness, and end of life issues. Coordinate with Palliative Care and provide information and education to patients/families for completion of Advance Directives, POLST, and determine goals of care. The Oncology Social Worker is a strong advocate for patient/family rights with a strong knowledge base in the area of bio-ethics and efficacy of treatment. The position requires an individual experienced and/or willing to obtain certification in Palliative Care and Oncology Social Work Practice.
The Case Management/Social Work Department is composed of full time and part time MSW/LCSW staff with advance training and experience in assessment, intervention, and discharge planning activities in the acute care hospital setting.
