Job Description:
Responsible for providing social work services to Brazosport Regional Home Health patients and families. Acts as a liaison with medical staff, nursing, allied health professionals, community agencies and others. Demonstrates knowledge of the issues associated with individuals of particular age and development and applies this knowledge to practice. Interviews, assesses, and plans treatment interventions to address patient and family needs in the home. Provides resource to staff and patients regarding community resources, long-range planning and problem-solving. Evaluates patient homes, family interactions and coping strategies to meet patient care needs in the home. Assist patients and family in coping with the limitations of illness. Evaluates home/family situations in order to assess needs. Maintains a competent level of professional expertise through a program of personal and professional continuing education. Constantly aware of patient safety. Functions as resource to patients, families and staff members in relation to community resources. Adheres to agency policies and procedures, assists in fulfilling the philosophy and objectives of the hospital and department while maintaining confidentiality of information. Maintains accurate and complete patient care records. Maintains a professional manner at all times. Participates in agency wide performance improvement activities as indicated or assigned. 3 years experience in a clinical setting performing medical social work required. 5 years of medical social work preferred.

Education:
Graduation from accredited school of social work with a Master's degree MSW. Additional educational opportunities.

Training:
Social Work training in a medical/clinical environment. Excellent customer service skills. Training in a medical/clinical environment.

Special Skills:
Ability to respond to common inquiries or complaints from customers, or regulatory agencies. Ability to effectively present information to top management and public groups. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Excellent communication, conflict management, organizational and planning skills. Ability to provide own transportation to visits; drive within 50 mile radius of office on a daily basis.
