North East Medical Services (NEMS), a community health center in San Francisco is looking for a part-time Member Services Representative for our San Jose clinic. Under general supervision carries out eligibility screening for clients and prospective clients on a regular basis, assess the health and related needs of prospective clients and makes appropriate referrals, advises and counsels clients regarding a variety of personal matters, acts as an advocate for the clients and performs related duties as required.

Job Responsibilities:
Inform prospective clients of the services available through NEMS.

Determines the eligibility of prospective clients according to NEMS' guidelines and carries out the application process.

Conducts periodic review of the eligibility and active status of the clients, and follow up on clients who are not utilizing project services.

Interviews clients regarding their health and related problems and makes referrals to other NEMS' services and community health agencies as appropriate.

Updates client information and data as it occurs, i.e. change of address, name, additions, etc.

Enters necessary data in computer to complete the registration/re-certification process.

Advises and counsels clients regarding family, economic, physical and emotional matters. Provide assistance in resolving these matters whenever appropriate.

Confers with Social Work Supervisor in evaluation of the client's needs and problems.

Maintains needed data on clients, record in clients' medical records and prepares statistical reports as required.

Performs duties as assigned by supervisor and President & CEO.

Marginal Job Functions

Maintains up-to-date records regarding eligibility and related data.

Helps and escorts clients applying to Medi-Cal, Medicare or other financial assistance according to need.

Attends and participates in community meetings and programs when appropriate.

Qualifications:
Bachelor degree in Social Work and two years of work experience in social work or related field.

Experience and/or training in medical social work desirable.

Understanding of human behavior as related to interpersonal relationships.

Ability to interview people of all ages and socio-economic levels effectively.

Bilingual (Cantonese/English)and knowledge of other Chinese dialects desirable.

Knowledge of state, city and community social services agencies helpful.

Ability to type 45 wpm.

Computer experience an asset
