The Mental Health/Substance Abuse (MH/SA) Care Coordinator Clinician - CCNC is responsible for providing leadership in Cardinal Innovations' collaboration with local networks of the Community Care Plan of North Carolina (CCNC). This Incumbent is responsible for ensuring accurate and timely Care Coordination for identified individuals to ensure that these individuals receive appropriate assessment and services. This is an office based position, though work is done in a variety of locations.

The MH/SA Care Coordinator Clinician – CCNC is responsible for (though not limited to):

Leadership in Cardinal Innovations-CCNC Collaboration

Represents Cardinal Innovations Healthcare Solutions (Cardinal Innovations) at CCNC network meetings

Serves as a collaborative partner in identifying system barriers through work with local CCNC networks

Tracks Care Coordination referrals and follow-up, maintaining needed documentation

Collaborates with Community Operation Center Care coordination Managers to ensure ongoing interdisciplinary team staffing with CCNC networks

Provides regular updates and summaries to Regional MH/SA Care Coordination Manager regarding collaboration with CCNC networks

Develops, updates and maintains process maps for behavioral health integration with CCNC Networks and Cardinal Innovations waiver expansion sites/Community Operations Centers

Stays abreast of new CCNC initiatives and provides updates to Regional MH/SA Care Coordination Manager

Ensures consistent processes across Cardinal Innovations waiver expansion sites and CCNC Networks

Clinical Consultation, Planning and Training

Proactively ensures that individual identified as a Special Needs enrollee that has treatment needs or needs regular monitoring has a Behavioral Health Clinical Home and a Medical Home

Assists in educating others within Cardinal Innovations about the Community Care Plan of North Carolina (CCNC), Cardinal Innovations’ collaboration with local CCNC networks, and resources/tools available to assist in coordinating care (e.g. Pharmacy Home)

Provides technical assistance as needed to support Cardinal Innovations waiver expansion sites in developing and maintaining local collaborations

Develops protocols to facilitate dissemination of Provider Portal information to Cardinal Innovations provider network

Ensures CABHAS are trained in integrated care and follow Cardinal Innovations-CCNC directed initiatives

Collaborates with Medical Director and Cardinal Innovations waiver expansion sites on co-location opportunities

Measures results of intervention and treatment, including reduction in high risk events and inappropriate service utilization

Ensures that services for the individual are coordinated across the Cardinal Innovations system and with other systems, including primary care

Managed Care Treatment Planning Care Coordination

Risk Management – Proactively ensures that individual identified as a Special Needs enrollee that has treatment needs or needs regular monitoring has a Behavioral Health Clinical Home and a Medical Home

Ensures that a Person Centered Plan (PCP) is developed by a Behavioral Health Clinical Home or, if necessary, by the Care Coordinator to meet urgent needs and to access care for the individual

Convenes key providers and others to address needs of the individual, including the Targeted Case Manager if applicable

Identifies gaps in services and intervenes to ensure that the individual receives appropriate care

Measures results of intervention and treatment, including reduction in high risk events and inappropriate service utilization

Ensures that services for the individual are coordinated across the Cardinal Innovations system and with other systems, including primary care

Provides clinical discharge planning assistance to local hospitals and tracks individuals discharged from state and local hospitals to ensure they follow up with aftercare services, and receive needed assistance to prevent further hospitalizations.

Desired Skills & Abilities

A high level of diplomacy and discretion is required to effectively negotiate and resolve issues with minimal assistance. This will require exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts. Problem solving, negotiation, arbitration and conflict resolution skills are essential to balance the needs of both internal and external customers. Must be highly skilled at shifting between macro and micro level planning, maintaining both the big picture and seeing that the details are covered.

MH/SA Care Coordinators Clinician must exhibit an extensive understanding of the Diagnostic and Statistical Manual of Mental Disorders (current version) and have considerable knowledge of the MH/SA/DD service array provided through the network of Cardinal Innovations providers. Additional knowledge in Cardinal Innovations’ Medicaid B and C waivers and NCQA accreditation is essential.

The incumbent must be detail oriented, able to organize multiple tasks and priorities, and to effectively manage projects from start to finish. Work activities quickly change according to mandated changes and changing priorities within the department. The incumbent must be able to change the focus of his/her activities to meet changing priorities. Proficiency in Microsoft Office products (such as Word, Excel, Outlook, PowerPoint, etc.) is required.

A masters’ degree in a human services field (such as Psychology, Social Work, etc.) is required, along with at least two years of post-degree progressive experience providing similar services to the population served.

A valid licensure or certification in profession (social work, counseling or psychology) is required.

Working Conditions

This is a community-based position which primarily occurs during normal business hours. Cardinal Innovations reserve the right to call upon this employee during non-business hours as needed to meet the demands of the position.
