This is a Full-time & Periodic positions available position.

Location Charlottesville, VA

Qualifications Bachelor’s level and one year of service experience required
Direct support experience with both developmental disabilities and mental health populations preferred
Experience with crisis prevention and intervention services preferred
Maintain valid driver license and insurance for personal vehicles utilized for work
Complete within specified time period the trainings as required by agency and service model

Responsibilities The Mobile Crisis Counselor provides support to persons with co-occurring developmental disabilities and mental or behavioral health needs who are temporarily accessing emergency or planned respite in the individual’s home
This staff participates in the daily routine of the home and provides support for therapeutic, habilitative, personal care, health care and other activities
The Mobile Crisis Counselor participates as part of comprehensive REACH services that collaborate with the individual’s primary provider and clinical home, community crisis services, family and others
Provide a therapeutic environment to address guests, personal care, behavioral health, wellness and leisure activities needs
Establish rapport with guests living with intellectual disabilities and challenging behaviors while maintaining respect for each guest’s dignity and individuality. Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports in accordance with crisis guidelines and the person’s plan
Plans, structures and prompts social and leisure time activities, including activities that build skills for behavior management, stress management, communication, adaptive living and daily living
Maintains timely documentation of progress notes and other data, including incident and accident reports, behavior tracking notes, group notes, medication administration records, etc.
Responds to individual safety needs, including reporting significant changes in behavior and health
Adheres to program standards and policies as well as local, state and federal regulations
