Coordinates the HIV specialty and primary care clinical activities for and provides nursing case management services to patients who are seen in the Moore (outpatient HIV) Clinic from the initial visit until termination of services. Independently assesses daily laboratory reports for abnormalities; communicates abnormalities to medical provider and implements intervention plans when clinically warranted in collaboration with provider. Provides clinical triage and crisis intervention during clinic hours, responds to acute patient calls and drop-in visits, clinically assess need for patient diagnostic testing prior to evaluation by medical clinician and facilitates and implements all interventions required in collaboration with provider. Counsels, supports, educate, and coordinate resources needed by patients and their family members during each stage of HIV disease and primary care related conditions (e.g., diabetes and hypertension) as clinically and medically warranted.

Oversees case management activities and coordinates care of home care nurses and facilitates communication between family, home care nursing, physician, and patient.

Requires Baccalaureate degree, or Associate degree, or Diploma in nursing from an accredited School of Nursing and eighteen months of experience as a registered nurse. Responsible for coordinating care for a group of patients for a designated time period to achieve clinical and organizational outcomes. Provides direct patient care based on the nursing process; provides direction to/manages other team members to promote quality and excellence of care. Works collaboratively with a multidisciplinary health care team to optimize team performance. Promotes improvement in unit processes and systems to benefit patient care and achieve unit goals. May act in the absence of the Nurse Clinician (NC) III to oversee daily care for a group of patients, as assigned. Candidates must be eligible for Registered Nurse licensure in the state of Maryland. Knowledge of nursing process. Social, behavioral and physical knowledge of current health care environment. Highly effective verbal communication and interpersonal skills to establish working relationships that foster optimal quality patient care. Ability to work in a fast paced university teaching hospital.
