This position is located in Chula Vista
Variable 8-hour shift, working every other weekend
MS in Nursing for Nurse Practitioner required
Current unencumbered California Registered Nurse licensure required
Certified Adult or Family NP required
Certified diabetes educator (CDE) or ADM preferred
DEA License required or within 6-12 months of hire
Experience caring for diabetes patient population in the inpatient or outpatient setting strongly preferred
Department willing to train the right candidate
Bilingual (English/Spanish) preferred
Assists inpatient physicians and endocrinologist in managing the care of the Diabetes patients at Sharp Chula Vista Medical Center. Collaborates with medical team of intensives, hospitalists, endocrinologists and other health team members to ensure maintenance of Diabetes patient care standards and continuity of care. Performs inpatient extended role procedures as defined by Standardized Procedures to facilitate the timely and appropriate care process of the Diabetes population. Must provide patient assessment and documentation. Excellent communication and time management skills. Collaboratively sets parameters and guidelines for continuum of care with the multidisciplinary team. Recommends and coordinates follow-up, consults, and referrals. Educates patients and families regarding expected course for inpatient stay. Participates in continuing education programs, current literature reviews to maintain and upgrade skills and knowledge. Participates in quality improvement programs and utilization management.
