health outcomes. through preventive care and screenings, and intensive care management of the frail and chronically ill.

Location: Mason, OH WellPoint office

Hours: 8:00 am – 5:00 pm EST Monday - Friday

As an LPN/LVN Nurse, you will serve as a nurse-extender coordinating the delivery of care for our Medicare Advantage frail and chronically ill members.

During the initial launch of the program, primary duties may include, but are not limited to:

  • Implements, coordinates, monitors, and evaluates options and services to meet members health needs and ensures appropriate use of clinical resources.
  • Expedites and coordinates appointments for assigned members.
  • Keeps all providers involved with member’s care updated on appointments, condition, and additional clinical support needed.
As program expands, may perform the following duties:
  • Reviews patient medical records, interviews patients and measures vital signs.
  • Explains procedures, performs treatments, and administers prescribed medication.
  • Prepares treatment room; assists physician with procedures; cleans and sterilizes equipment.
  • Performs venipuncture, urine dipstick, blood glucose testing, PT/INR, HgB, wound culture, foley catheter care and insertion, enteral tube feeding and care.
  • Facilitates patient transfers to alternate level of care.
Job Qualifications

Requires an LPN or LVN; 2 years of experience as an LPN/LVN; or any combination of education and experience, which would provide an equivalent background.

Current LPN/LVN license in Ohio required.

Must have strong oral, written and interpersonal communication skills, PC skills to include word processing, spreadsheet, and database applications, organizational and problem-solving skills, and decision-making skills.

Travel between different care settings required up to 75% travel limited to specific assigned geographic area, typically within a 40 mile radius. Mileage will be reimbursed.

May be required to work some corporate paid holidays.
