Primary Care Extender is an Advanced Practice Nurse or Physician Assistant that has completed the course study of said title and obtained the licensure requirements for the state of Nevada. An extender in the Southwest Medical Assoicate, Inc. Clinics has documented clinical practice experience to assess patients of various age groups and has ability to diagnose, create treatment plans and prescibe medications as related to the treatment plan. The extender develops plans in collaboration with a physician to achieve the best health outcomes for the patient in their primary care team. Exposed to patients with potential infectious disease. Normal Clinical care lifting and provider office environment.
Responsibilities for Primary Care Nurse Practitioners include:
-Rapidly formulate a differential diagnosis from assessments conducted within the clinic setting
-Develop and implement treatment plans for patients assigned within the primary care team.
-Develop and actualize plans for the primary care team as assigned
-Provide guidance to nursing personnel towards treatment plans and care issues.
-Documents treatment plans in the medical record.
-Documents collaborative guidance from the physician as needed.
-Coordination of care for patients
-National Nurse Practitioner certification
-Certification in Adult, Family or Gerontology
-Licensure in the State where this position is based, or ability to obtain
-Ability to communicate with patients, primary care team members and management staff.
-Ability to make care decisions based on the best medical practices without direct supervision or guidance by a physician.
-Intermediate level of technical proficiency in the use of computer programs for medical records or internet search for clinical practice.
-Strong acute care experience in a primary care clinic, urgent care, or retail clinic
-Ability to formulate a differential diagnosis.
-Proficient in assessment and development of treatment plans.
-Ability to conceptualize the needs for the team or department and actualize plans for implementation.
-Empowered to develop plans of action and gain team consensus towards common goals.
-A background in managed care
-Familiarity with ICD-9 coding
