Physical Therapists - Reports to the team coordinator. Assumes responsibility and accountability for a designated group of patients and accurately provides Physical Therapy (PT) evaluations; develops and implements PT treatment plans; provides education and information to patients and caregivers; participates in required discharge planning and completes all required documentation according to policy. The PT supervises and assumes responsibility for are provided by Licensed Physical Therapy Assistants. May be assigned other duties such as clinical instructor for student affiliations, performance improvement, and program development. Must have a Bachelor or Master Degree in Physical Therapy from an American Physical Therapy Association accredited program. Must have a current license as a Physical Therapist in the state of Virginia or be licensure eligible. Other requirements include-BLS completion within 3 months of hire. Must have excellent written and oral communication skills; ability to operate and apply therapeutic equipment; ability to lift/push/pull light to heavy equipment, supplies, and patients; creativity and problem solving skills.

Productivity:
Weighted Visits
o Admission = 2 Credits
o Eval/Resumption of Care = 1.5 Credits

Expectations:
o 30 Credits/Week (5-7 Credits/Day)
o Every 2 weeks, a bonus per visit rate ($45-60 per visit rate) paid for every credit over the set productivity expectation for that 2 week period
 ex. 31 Credits in Wk 1 + 30 Credits in Wk 2 = 61 Credits = Bonus
 ex. 31 Credits in Wk 1 + 29 Credits in Wk 2 = 60 Credits = NO BONUS


Mileage:
Reimbursement starts from office or 1st patient home
Reimbursed at IRS Rate
Average 40-60miles/day
o If exceed 100miles/day then that counts as a credit towards the 30 Credits/Wk
