Position is on-call with varying hours and shifts. Various opportunities to pick up extra hours as available. With successful performance, on-call employees may be considered for positions with more permanent hours.
This position is provides each resident with a safe environment, assists with activities of daily living, gives emotional and social support and attends to their physical comfort.
Examples of Duties:
Provide personal hygiene including bathing (tub, shower, bed bath), oral care, skin and nail care and hand washing.
Complete resident grooming including hair care, shaving, dressing and weighing. Encourage residents toward independent personal care.
Assist with toileting including bowel and bladder retraining and perinea care.
Perform and assist with mobility, transferring, ambulation, positioning and range of motion per the care plan.
Carry out Universal Precaution procedures and isolation techniques.
Assist in orienting residents to household environment and routines upon admission and throughout stay as needed.
Document accurately all care given, including dietary intake and output, temperature, weight, blood pressure pulse and respiration, using approved abbreviations and descriptive terms.
Maintain confidentiality of all resident care information and assure resident’s rights and privacy are protected at all times per HIPAA.
Report to nurse on units for written and verbal assignments which include daily use of care plans for assigned residents before initiating care.
Participate in the planning of resident care through shift reports and resident care staffing meetings when assigned.
Inform verbally and/or submit in writing any observations and changes in a resident for the licensed nurse to evaluate including dietary intake.
Serve residents their meals and motivate and assist them with intake of nourishment and fluids.
Answer all call signals and requests of residents immediately.
Encourage/assist residents in keeping their rooms clean and orderly.
Maintain nursing unit in a clean and orderly manner.
Keep all residents’ personal belongings and equipment clean.
Operate all equipment in a safe manner.
Escort residents to appointments and/or therapy departments as scheduled.
Participate in unit activity programs planned for residents as needed.
Attend and participate in unit nursing staff meetings and mandatory in-services.
Perform a total inventory of all resident’s possessions on admission, discharge or transfer.
Send all residents’ possessions to laundry (clothing) and/or Medical Supply clerk (dentures and glasses) for labeling.
Respond to resident emergency including bleeding, falls, seizures and performance of Heimlich maneuver.
Assist with post-mortem care as assigned.
Transport dietary carts.
Transport linen carts to laundry.
Create and maintain an atmosphere or warmth and personal interest as well as a clean environment throughout the unit.
Work tactfully and cooperatively with residents, families, visitors and entire staff throughout the organization.
Perform other duties as assigned.
Skills, Abilities & Knowledge Required:
Ability to verbally communicate with residents, families, the public and other members of the organization. Must be honest, dependable, exhibit a warm, cheerful caring manner. Must be able to safely perform the essential job functions. Must be
able to be reached by telephone. Must be able to read, write, speak and understand English. Prefer basic computer skills.
High School or equivalent. Successful completion of CNA training course.
Minimum of 16 years old. Any equivalent combination of experience and training which provides the required skills, knowledge, and abilities. The incumbent must be able to lift and carry up to 50 lbs. If more than 50 lbs. is to be lifted, then it needs to be a 2-person lift or aided by a mechanical lift.

Wisconsin certification as a Certified Nursing Assistant and on state registry.
