The Anoka County Community Health and Environmental Services Department has openings for two full-time (1.0 FTE) Public Health Nurses for the Family & Community Health Unit who will plan, implement, and provide direct services to clients and families. These positions include provision of services to low income, high-risk pregnant women, including adolescents, parenting, chronically ill/disabled children as well as community outreach and prevention efforts. These positions are limited term contingent upon funding through March 20, 2015.
EXAMPLES OF ESSENTIAL DUTIES (ILLUSTRATIVE ONLY):
Through evidence-based practice provides assessment, counseling, health education, and follow-up to Family Health client caseload, including high risk pregnant women/adolescents and parenting families/adolescents, and chronically ill/disabled children; Provides family planning information and counseling to families and individuals as needed; Assists young families with children by providing information and guidance on growth and development, social and emotional difficulties; Develops nursing care plans by matching patient/client needs with professional abilities and provides or delegates the needed care; Consults with other professionals relative to individual client or family health problems; Refers program participants to community agencies, health providers and social services; Prepares and maintains pertinent records and reports; Participates in staff meetings, team meetings and case conferences as appropriate; Assists in coordinating program services with WIC, Public Health Nursing and Social Services staff; Collaborates with other community agencies such as schools, health providers and others to meet the needs of the high risk Family Health clients; Serves as a resource to the agency and community groups regarding maternal and child health concerns; Assists in developing nursing protocol for management of high risk pregnancy conditions and chronically ill/disabled children; Assists in developing and implementing data collection procedures for the purpose of program reporting and evaluation; Assists in evaluating program outcomes; Participates in immunization clinic activity and Direct Observe Therapy home visit activity; Represents the agency to the public, participates in educational presentations, and participates in ongoing program outreach. Participates in Emergency Preparedness activities and other department activities as assigned.
Bachelor's Degree from a school of nursing accredited by the National League of Nursing; current licensure as a Registered Nurse in the State of Minnesota plus Public Health Nursing Certification; one year of related experience in Public Health Nursing or highly related area; or equivalent combination of education and experience.
Knowledge of: Public Health Nursing principles and practices; adolescent health issues; health and disease processes and nursing interventions; community resources available to the patient.
Ability to: Work with clients and their families; meet and deal with the public; teach and work with paraprofessionals; make decisions and exercise independent judgment; prepare and maintain accurate charts, records and reports; develop a plan of care for a client; determine when ancillary services for clients are required; lift and carry with ease nursing equipment and supplies; lift and provide physical assistance to patients as needed.
Skill in: Written and oral communication.
Must have: Own transportation for business use (e.g. home visits, attend meetings at various sites, etc.).
Must successfully complete driver's license and criminal background checks.

Normal hiring range: $26.37- $29.46 per hour depending on qualifications.
Salary range: $26.37 - $38.71 per hour.
