We have an opening for a Patient Advocate/Health Care Representative for our Kirkland hospital location. This position is fulltime, Monday through Friday with the work hours 2:00 PM to 10:00 PM. Some of the duties include:

  • Screening patient referrals
  • Reviewing patient hospital charts
  • Interviewing patients on the phone after discharge and face-to-face while still in the hospital
  • Completing applications, by hand and on computer, for state and federal programs
  • Assisting and assessing the needs of the patient and hospital to ensure that referrals are resolved timely and appropriately
  • Providing updates and assistance to hospital personnel and other staff as needed
  • Maintaining documentation in both hospital computer and company database
  • Bachelor's Degree
  • Knowledge of State Medicaid (ABD, TANF, Family Medicaid) and/or SSI/SSDI disability application processes and programs
  • Ability to read and apply the information contained in medical records to appropriate program
  • Previous case management experience
  • Ability to apply knowledge of state/federal programs to benefit coverage for the patient
  • Reliable transportation; sometimes a local home visit may be needed
  • Understanding of and the ability to apply the medical definition of disability as it relates to body systems and functions of daily living
  • Demonstrated experience working with people of various educational and socioeconomic backgrounds
  • Computer skills, including word processing and data entry
  • Experience with both Microsoft Word and Excel programs
  • Demonstrated strong written and verbal communication skills
  • Fluency in other languages: Spanish, Korean, Mandarin Chinese, Polish, etc
  • Previous Hospital business office experience in Financial Counseling or Admissions
  • Previous experience in Social Work, Nursing and or Utilization Review
  • Previous experience working for Social Security Administration (SSA), Department of Social and Health Services (DHHS) and/or Washington Department of Health
  • Certification as an ACA Navigator, Assister or Application Counselor
  • Knowledge or experience with Washington Health Plan Finder
