SUNY Downstate Medical Center is seeking a Patient Navigator to provide field-based health education on a monthly or weekly basis and skills building service to patients.

Duties:
Approximately 75% of this position’s duties take place in patients’ homes or the community. Accompany patients to health care appointments and social service encounters. Implement health education curriculum on a one-on-one basis with a caseload of 15 -18. Coordinate appointment keeping reminders and set up patient transportation. Conduct social service reassessment and follow-up. Provide critical feedback to other members of the health care team based on observations in the field through electronic medical record documentation and participation in multidisciplinary care conferences. Educate, coach and empower patients. Provide directly observed therapy (DOT) in patients' homes as needed; assess for and report any antiretroviral therapy related side-effects. Document all patient activities on required data forms and in patient medical record.

Skills

Requirements: AA or AAS degrees with 1 to 3+ years of experience providing case management, social services or health care services required. Bachelor’s degree and experience with home visits preferred. Bilingual in English/Haitian-Creole or English/Spanish preferred. Experience working with individuals living with HIV and/or co-occurring substance use and mental health disorders strongly preferred.
