Job Summary: Navigators provide a variety of services such as health education, outreach, and basic health assessment of people who are at disproportionate risk for cardiovascular disease. They also assist these persons in accessing health care. Routine responsibilities include screening, documentation, and accountability for meeting program goals and objectives. Additional tasks include: developing relationships with Community-based organizations; promoting cardiovascular risk factor screening; navigating to primary care and community based lifestyle resources as well as support of individual behavior change.
The Patient Navigator position will be a member of the Health Promotion Division of Denver Public Health. This position will conduct activities of the “Colorado Heart Healthy Solutions” project, a community outreach position funded by the Colorado Prevention Center. The position is part of a statewide chronic disease prevention program to reduce the burden of cardiovascular disease in at-risk populations.
This position will function as a member of a team of people committed to health and wellness for the residents of Denver and beyond.
Under general supervision, provides basic care coordination, identifies and reduces barriers to care, provides referrals to necessary resources.
Education: High School Diploma or GED.
Experience: Typically requires at least one year of community involvement and/or working with clients, or an equivalent combination of education and experience.
Knowledge, Skills & Abilities: Knowledge: Basic care coordination, knowledge of community resources Skills: Establish and maintain rapport and a therapeutic relationship, Communication and motivational interviewing, Uses computers, including Microsoft Outlook, Word, PowerPoint, Excel, SharePoint, GoldMine, scheduling software as well as perform Web searches for clients, BLS. Abilities: Bilingual English/Spanish or English/other languages preferred.
Certificate/License/Registration: Certificate of completion from the Community College of Denver Community Health Worker program and/or the Patient Navigator Training Collaborative at the University of Colorado, or an equivalent combination of education and experience.
