(3) Regular, Full-Time, RCUH Non-Civil Service position with the Pharm2Pharm project, funded by the Centers for Medicare and Medicaid Services (CMS) Innovation Center and coordinated through the University of Hawaii (UH) at Hilo College of Pharmacy Center for Rural Health Science, located at Hilo, Hawaii. This position will be working on Hawaii island (Kona). Continuation of employment is dependent upon program/operational needs, satisfactory work performance, availability of funds, and compliance with applicable Federal/State laws.

MINIMUM MONTHLY SALARY: $4,569/Mon.

DUTIES: The goals of this project include improving access to care, patient satisfaction, and medication safety, while reducing overall costs of care and building a workforce pipeline and payment model to sustain successes. Coordinates and contributes to the Pharm2Pharm service model. Works in an interdisciplinary team to facilitate screening, medication reconciliation, patient education, and monitoring of patients receiving services in the Pharm2Pharm project as needed in the designated region. Supports patient care services in accordance with the current Pharm2Pharm model/manual, which will be updated as needed, with the goal of improving the quality of care, improving patient health, and reducing total cost of care. Supports proper documentation of the patient care provided in accordance with the Pharm2Pharm model/manual and project requirements. Participates in project-wide training, related meetings and activities, to continuously improve the effectiveness and efficiency of the model.

PRIMARY QUALIFICATIONS: EDUCATION: Doctor of Pharmacy (PharmD) Degree from an accredited college or university. EXPERIENCE: At least one (1) rotation (or six (6) weeks) of clinical experience in the Pharm2Pharm service model. ABIL/KNOW/SKILLS: Knowledge of prevention of adverse drug effects and interventions to improve patient compliance with medications. Knowledge of medication safety in the elderly. Able to work with primary care patients and inpatients, conduct patient education, gather appropriate information, and make appropriate recommendations. Proficiency in Microsoft Office, including Word and Excel; use of electronic drug reference sources. Effective oral and written communication skills.

SECONDARY QUALIFICATIONS: At least two (2) years of experience working/training in rural settings with the elderly and/or other high risk populations. Working/training experience with initiatives designed to improve quality and reduce cost of care.
