FT - Coordinates care center staffing and clinical services. Oversees and participates in direct patient care to include clinical services and order entry as determined by activity and volume in the care center. Education and formal training with Doctor of Pharmacy Degree (PharmD) from an accredited college of Pharmacy, clinical pharmacy residency or equivalent exp and licensure by NC Board of Pharmacy. Two yrs as a clinical pharmacist with involvement in formulary development, drug use evaluation, and quality assurance activities req. Transplant pharmacists needed for liver, kidney and heart transplant programs. Round with multidisciplinary team and provide guidance on immunosuppression and transplant management. Attend weekly team meetings to participate in discussions on patient care plans and policy/protocol development. Participate on QAPI teams to help improve transplant care to all patients during pre, intra and post phases. See inpatients to provide education on transplant medications and care plan and outpatients to provide oversight of medicinal regimens pre and post transplant. Provide education to hospital staff on caring for transplant patients. Work with physician on drug studies and help provide patient education and oversight for research. Transplant experience or training strongly preferred. Transplant residency completed or equivalent experience strongly preferred.
