The Project Manager - Clinical Transformation manages the portfolio of initiatives within the Department of Quality Management. These initiatives are designed to improve patient outcomes at all UHS Acute Care facilities. Leads the development, planning, and execution of time-lines to improve patient outcomes. Works collaboratively to implement the time-line effectively and efficiently. All initiatives are in alignment with the organization’s vision and strategic plan goals to be the Safest Health System in the Nation and the National Benchmark in Clinical Outcomes.

Essential Job Duties:

Facilitates clinical improvement projects focused on implementation of evidence based medicine, chronic disease management, complication and readmission reduction, and access and coordination of patient care. Leads the development of annual improvement goals for their assigned projects and teams (process and outcome measures) that are in alignment with the organizations vision and strategic plan goals to be the Safest Health System in the Nation and the National Benchmark in Clinical Outcomes.

Ensures that CQI teams are multidisciplinary in membership, and utilize data driven, rapid improvement methods to maintain a rapid pace of improvement. Ensures that team membership includes a large representation of physicians who are well respected and considered opinion leaders by their peers. Ensures that all teams are co-lead by a medical director and nursing leader.

Supports the Directors of Clinical Excellence and Effectiveness by providing research in evidence based medicine and nursing practice, population health management, chronic disease management, complication and readmission reduction, construction of order sets, EMR/CPOE rules and alerts, design of practice support tools, physician engagement strategies, change of physician practice patterns, use of clinical dashboards, rapid cycle redesign, process improvement, project management, and change strategies. Supports the patient family experience work by providing research in service excellence, service recovery, HCAHPS dashboards, and best practices to exceed the patient-family expectations against needs.

Supports implementation of nationally endorsed EBM guidelines shortly after publication. Is familiar with and can navigate Zynx Health Website. Supports the implementation of vendor based products and services to improve patient outcomes.

Partners with Quality Analytics Department members to support CQI teams with clinical dashboards and other measurement tools. The PM and the data analyst form a unified support team for each project with each having distinct areas of leadership and responsibility to the team.

Tracks UHS external ratings by external rating agencies (Health Grades, Leapfrog, and Thomson Reuters, among others) and regulatory agencies. Recommends improvements to maintain highest ratings (e.g. 5 stars) and rankings (e.g. Top 100 Hospitals, Hospital Compare, among others). Within their scope of responsibility ensures that all medical conditions or surgical procedures evaluated by external agencies have active CQI teams. The PM supports these activities with yearly project plans outlining measureable improvements and quarterly milestones.

May lead innovation projects that redesign current care process aligned with the domains of the patient family experience such as pain management, communication, cleanliness/quietness, or the discharge process. In such instances the PM will likely be paired with an operational/process improvement specialist to accomplish the work. In addition, an effective and collaborative working relationship with care management will often be required to accomplish team goals.

May assist the Quality Directors with grant applications and award submissions, participating in the orientation of new employees, leading the design of quality related presentations and storyboards from their assigned improvement activities, as requested by leadership.

Participates in patient safety initiatives and risk mitigating projects.

Performs other duties as assigned.

Requirements

Bachelor’s degree in areas including, but not limited to, health care, engineering, case management, or business administration required. Master’s degree preferred.
Five years experience in healthcare or manufacturing, quality improvement, care management, or equivalent combination of education, training and experience.

Clinical background desired, but not necessary. Previous project management and facilitation experience required including demonstrable successes. Experience working in an organization of size and complexity comparable to UHS. Demonstrated track record for implementing and guiding large projects and teams.
Advanced knowledge and ability to teach clinical, operational, and process improvement methodologies and tools. Advanced knowledge of healthcare regulations, accreditation guidelines and standards. Superior leadership, team facilitation, change management and teaching abilities. Good working knowledge of statistics, and must be expert in the use, application, and interpretation of statistical process control charts. Must have a level of proficiency in medical coding that ensures accurate interpretation of clinical dashboards with the ability to explain measure definitions to the satisfaction of physicians, nurses and other clinical professionals. Quickly attains an advanced working knowledge of academic detailing techniques. Utilizes these techniques to ensure effective communication to all team members and physicians operating in their assigned specialty areas.
Ability to work effectively with various levels of leadership, management, physicians, and other community members. Advanced skills in leading and facilitating groups, decision-making and conflict resolution. Ability to manage multiple priority projects simultaneously. Able to work flexible hours as needed.
